Darlington 90 bed aged care facility.
We have a permanent Part Time position available for a Registered Nurse experienced in ACFI coordination. In addition to regular floor shifts this role will be rostered two days per week for ACFI coordination duties.
Be part of our team of clinical professionals and provide contemporary evidence-based nursing across all levels from high-care, low-care, palliative and respite services to our aged residents.
This position includes: - Quality care in accordance with current nursing care plans
- Assessment, prevention, early detection, referral and health promotion
- Formulation of care plans, reviews and case conferencing
- Ensuring that care plans reflect the true care needs of the customer and support accurate ACFI funding
About You: Your background within aged care and ACFI documentation will see you settle in well to this position. Flexibility to work various shifts such as business hours, afternoons and weekends is highly beneficial. We also require:
- Current registration with AHPRA as a Registered Nurse
- Demonstrated knowledge and skills in ACFI coordination is essential
- Aged care experience is highly desirable
- Prior experience in nursing care and clinical assessment
- Medication administration and wound management experience
